Gateway Hospital – Payroll Problems Follow Years of Unpaid Property Taxes

GRANITE CITY, IL – August 6, 2026 (STL.News) Financial troubles at Gateway Regional Medical Center have deepened after payroll delays affecting employees were followed by the disclosure that the hospital’s owner has accumulated years of unpaid property taxes since acquiring the facility in 2023.

Madison County property records show that Los Angeles-based American Healthcare Systems Illinois has failed to pay property taxes on the hospital campus since purchasing the property. The unpaid tax obligations now total several million dollars, adding to concerns about the financial condition of one of the Metro East’s largest hospitals.

The tax revelations come just days after employees reported delays in receiving their paychecks, prompting questions from staff, patients, elected officials, and state regulators.

Gateway Hospital – Millions in Property Taxes Remain Outstanding

According to Madison County tax records, American Healthcare Systems Illinois owes property taxes on Gateway Regional Medical Center and several adjacent parcels associated with the hospital campus.

For the current tax year alone, the company owes more than $932,000, with payment due under Illinois property tax deadlines. Previous years’ unpaid balances have continued to accumulate, pushing the total owed into the millions.

County records also indicate that taxes remain delinquent on additional properties surrounding the hospital, suggesting the financial difficulties extend beyond the primary medical facility.

Gateway Hospital – Employees Report Delayed Paychecks

Earlier this week, Gateway Regional employees reported that scheduled payroll deposits failed to arrive on time. Some workers also said their paper paychecks could not be cashed because sufficient funds were unavailable.

Hospital administrators acknowledged the issue in a letter to employees, describing the disruption as a temporary cash flow problem. Leadership apologized for the hardship and thanked employees for continuing to provide patient care while the matter was being resolved.

Gateway Regional Medical Center employs more than 600 people and operates as a 343-bed hospital serving patients throughout southwestern Illinois and the greater St. Louis metropolitan area.

State Health Officials Visit Hospital

The Illinois Department of Public Health confirmed that inspectors visited Gateway Regional Medical Center following reports of the payroll issues.

The department also inspected another hospital operated by American Healthcare Systems as regulators reviewed the company’s operations. State officials have not publicly announced whether any deficiencies or violations were identified during the inspections.

Hospital services have continued during the state’s review.

Hospital Owner Responds

American Healthcare Systems CEO Michael Sarian said the company has been working to address the outstanding property tax obligations.

According to Sarian, new management has either paid delinquent taxes or entered into installment agreements to satisfy the debts. However, Madison County’s publicly available tax records continued to list unpaid balances as of the latest update. Property tax databases may not immediately reflect recent payments or approved payment plans.

Previous Hospital Closure Draws Attention

Gateway Regional’s owner previously attracted attention after acquiring St. Alexius Hospital in south St. Louis. That hospital permanently closed in 2023 following prolonged financial difficulties, ending more than a century of healthcare service to the community.

The earlier closure has increased public scrutiny as Gateway Regional now confronts payroll disruptions and significant unpaid tax obligations.

Community Impact

Gateway Regional Medical Center remains an important healthcare provider and one of the largest employers in the Metro East. Any prolonged financial instability could affect employees, patients, vendors, and surrounding communities that depend on the hospital for emergency and inpatient medical care.

At this time, hospital leadership has stated that patient services continue uninterrupted while efforts are underway to stabilize finances and resolve payroll and tax issues.
